Instructions

Please follow these steps in completing and submitting your Class Registration Form. This form is used to register students for Texas campus classes. The form must be filed at the CTC Records Office.

Enter your Legal name (Last name, First name, Middle initial).
Enter your social security number or student ID.
Enter your daytime phone number.
Enter your current physical address. Select yes or no for the change of address.
Enter your primary email address.
Enter your program of Study/Degree.
(Optional) This information is kept in strict confidence, has no effect on admission, and is used for tracking purposes only. Select the best choice.
Enter your state of residency.
Sign and date the form.
Enter the CTC Campus/Site Location, Term, and Year of the course.
Enter the Call Number, Course Name, Number and Section of the course.
