Students interested in the MLT program will need to submit an Application Packet to the MLT Office, located in Bldg. 150 (Science Center), Room 1114.

Monday-Thursday 7:30 a.m. to 5:30 p.m.
Friday 7:30 to 11:30 a.m.

The application packet will include:

  • Application
  • Verification of Policies
  • Verification of Receipt of Policies and Procedures
  • Acceptable Science Credit for Transfer to the MLT Program
  • Travel Acknowledgement
  • Dismissals
  • Criminal Background Check & Drug Screen Agreement
  • Student Health History
  • Personal & Family Health History
  • Physical Form - A physical exam must be completed on this form, and signed by attending doctor/nurse/PA
  • Two References - No letters of recommendation needed, please use these forms for references

Alongside with the application packet, students will need:

  • Immunization Records - to show all requirements within the appropriate dates
  • Transcripts (High School Diploma, GED or College Transcripts - unofficial copies are fine)
  • TSI Test scores, or exemption criteria met

Applications lacking any of the above criteria will be considered incomplete.

After complete applications have been received by the office, the student application will be reviewed by the program director and faculty, and students will be selected for acceptance into MLAB 1470. Acceptance is based on the date of submission of the complete application and available space. Up to twenty two (22) students are admitted to the MLAB 1470 courses per academic year.* After complete applications have been received by the office, students will be required to attend a program orientation.

Upon successful acceptance, students will be given a Letter of Acceptance, instructions for the mandatory orientation meeting, an MLT Degree Plan (MLAB5_AAS), and the necessary documents to register. (Students will not be able to register for any MLAB courses without these documents.)
