Evaluation Request

Instructions

To receive an evaluation of military and civilian credit you must:

1. have at least six semester hours with CTC with 2.0 GPA.
2. complete this form, sign it, and attach the appropriate documentation as specified below.
3. mail this signed form along with documentation to:
   

   Central Texas College
   Evaluations Office, Student Services Department
   P.O. Box 1800, Killeen, TX 76540.
