Requests for certificate/degree
and the fees for students from all locations must reach the Graduation Department on Central Campus by:
- October 1 for Certificate or Degree in December.
- February 1 for Certificate or Degree in May.
- June 10 for Certificate or Degree in August.
Requirements for Applying
- All course requirements must be completed prior to certificate/degree award. Requests for certificate/degree received from students who have not completed course requirements by the graduation date will not graduate.
- Continental & International students must file this form with the student service office at the campus they are currently attending or send the fee and application form by mail to

- Payment must accompany this application. If this application is filed by mail, payment must be by check or money order. No Cash Please!
- Associate Degree: $25.00
- Certificate of Completion: $25.00
