INSTRUCTIONS: HOW TO REQUEST ACCOMMODATIONS
*All DSS Forms must now be completed ONLINE. All DSS e-Forms can be found at https://ctceforms.ctcd.edu/index.aspx under ST Disability Services. The student is responsible for contacting a DSS Coordinator with any questions about the DSS e-Forms prior to completing and submitting the form(s)*
*IMPORTANT NOTE: If you are a student who is already registered with Disability Support Services (DSS) and currently or previously (within the last 3 years) received classroom accommodations through our office with your DSS Coordinator, you do not have to complete the Student Intake e-Form. Only new students who have not previously registered with DSS and/or have not received classroom accommodations from DSS within the past 3 years are required to complete the Student Intake e-Form.
Classroom Accommodations:
  • New students requesting classroom accommodations must complete and submit the Student Intake e-Form (this form only needs to be submitted once - this form does not need to be submitted every semester)
  • Students are also required to attach appropriate medical documentation as outlined in the Documentation Criteria Checklist to the Student Intake e-Form (if student is unable to attach the required medical documentation to the e-Form, the student is responsible for submitting the medical documentation either in person to the DSS office, via fax to 254-526-1700, or via email to a DSS Coordinator - students are responsible for following up with the DSS office once medical documentation has been submitted)
  • Accommodations will not be provided until the student has completed the Student Intake e-Form and required medical documentation has been submitted to DSS and approved by a DSS Coordinator
  • Once the student has completed the required DSS Student Intake e-Form and has had his/her required medical documentation reviewed and approved by a DSS Coordinator, the student is then required to complete and submit the Online Accommodation Request Form to formally request accommodations. Please use Internet Explorer when completing this form. Students are required to request accommodations every semester that accommdations are needed.
    • IMPORTANT NOTE: The Online Accommodation Request Form is currently in the process of being converted into an e-Form. If you are having any technical difficulties with this form, you may email your assigned DSS Coordinator directly to request your accommodations after completing course registration. Your email must include a list of ALL the courses and names of course instructors that you are requesting accommodations for. If your assigned DSS Coordinator is Troy Barber, please email him at troy.barber@ctcd.edu and if your assigned DSS Coordinator is Ron Porter, please email him at ron.porter@ctcd.edu. Campus Housing Accommodations:
  • Students requesting campus housing accommodations must complete and submit the Student Intake e-Form and Housing Accommodations e-Form (students are also required to complete the Housing Application with Morton Hall)
  • Students are also required to attach the appropriate documentation as outlined in these required e-forms
  • If the student is requesting classroom accommodations in addition to housing accommodations, the student must follow instructions listed above for classroom accommodations
Service Animal (SA) Accommodations:
  • Students who bring their service animals to campus are not required to register their service animal with DSS and are not required to submit medical documentation per disability laws.
  • However, students are encouraged to voluntarily register their service animal with DSS by completing and submitting the Service Animal Registration e-Form
  • Students who choose to register their service animal should attach the appropriate documentation as outlined in the Service Animal Registration Form (i.e., copy of veterinarian's verification that the dog has all legally required vaccinations to maintain the it's health and prevent contagious disease and a copy of the dog's license from the city/state in which it is registered)
  • If the student is requesting classroom in addition to the service animal, the student must follow instructions listed above for classroom accommodations. If the student needs to request campus housing accommodations with their service animal, the student must contact DSS.
Emotional Support Animal (ESA) Accommodations:
  • Registration of an ESA requires documentation of disability and an ESA is ONLY permitted in the student’s dormitory room only if the campus housing accommodation is approved by DSS (unlike a service animal, an ESA is not permitted in any other campus buildings)
  • Students requesting accommodations for an ESA must complete and submit the Student Intake e-Form, Housing Accommodations e-Form, and Emotional Support Animal Registration e-Form
  • Students are also required to attach the appropriate documentation as outlined in these required e-forms
  • If the student is requesting classroom accommodations in addition to the ESA, the student must follow instructions listed above for classroom accommodations
