You must complete the application below and submit a printed signed and dated copy to the CTC Department of Nursing to be considered for enrollment in the Preceptorship Program.
Instructions:
A red asterisk (*) indicates a required field.
Complete all required fields within the first three form tabs.
Click the Review Data and SUBMIT tab to verify all fields are correct.
If necessary, make changes in previous tabs and review again.
When all data is correct, click the "Submit My Application" button.
On the Congratulations! page, click the "Print My Application" button and print.
Sign and date the printed application.
Hand deliver, mail, or fax the signed/dated printed application to the CTC Department of Nursing.
