The following is REQUIRED in order to register. As it will take at least four months to complete the series of immunizations if you have not had any of the shots it is vital that you look into this NOW. THIS IS REQUIRED BY THE STATE AND YOU WILL NOT BE ABLE TO REGISTER in August UNTIL THIS IS DONE !
- ALL STUDENTS BORN ON OR AFTER 1-1-57 MUST SHOW PROOF OF:
- 2 doses of measles vaccine administered on or after their first birthday and at least 30 days apart.
- 1 dose of mumps vaccine administered after first birthday.
OR
PROOF OF IMMUNITY TO MEASLES AND MUMPS (based on blood tests which show levels of antibodies that confirm immunity or documentation from a physician who diagnosed the disease at the time it occurred).
- ALL STUDENTS REGARDLESS OF AGE ARE REQUIRED TO SHOW PROOF OF:
1 dose of rubella vaccine after first birthday or immunity to rubella (based on guidelines above).
- ALL STUDENTS MUST SHOW PROOF OF A DIPHTHERIA/TETANUS IMMUNIZATION WITHIN THE LAST 10 YEARS.
- The State Health Department requires the Hepatitis B Vaccine. This is a series of 3 injections which takes four months to complete. The total series must be obtained prior to registration. If you have an employer who offers this free and you have not started the series, DO SO. Further information about where to get the series at the lowest possible cost locally will be provided when you come for the NET Test.
- You must have a TB skin test (PPD) done within 90 days of starting classes and each year thereafter.
- You must provide documentation of having had Varicella (Chicken Pox) or the vaccine/titer.
A copy of your immunization record will be needed along with the physical. PLEASE LOOK UP YOUR PERSONAL RECORDS AND COMPLETE ANY NEEDED IMMUNIZATIONS IN A TIMELY MANNER!
