TRANSCRIPT REQUEST BY FAX

  • Please complete the CTC TRANSCRIPT REQUEST FORM 
  • Please fax in your request one time only. (Re-faxing your request can result in multiple charges to your credit card which will not be reimbursed.)
  • When using the CTC request form a cover sheet is not required.
  • Fax the form, with complete VISA, MasterCard, Discover, or American Express credit card information, type of card, card number, and expiration date to (254) 526-1111.
  • Our fax is a dedicated fax number and is open 24 hrs a day 7 days a week.
  • REMEMBER, the fax machine is usually busy, so if your fax does not go through the first time, please wait a few min. and try again.
  • Due to the high volume of requests received we do not confirm receipt of requests.
  • Please use black ink and press firmly when filling out the form, print/write clearly, neatly, and stay within the lines as anything written outside the lines may be lost in transmission.
  • Credit card processing can take an additional 5 business days to process before the transcript is mailed.
  • Requests will NOT be processed if:
    • Information is too light or small to read
    • Information is illegible—please type if possible or print clearly
    • Credit card number or expiration date is missing, incomplete, or expired.
    • Request is not signed by the student.
