To set up services with the DSS office for accommodations for college courses, the student is responsible and required to do all of the following (please read the webpage titled "Instructions: DSS e-Forms for Requesting Accommodations" for additional information):
  • Complete the admissions and registration process with the appropriate CTC departments.
  • Complete TSI Assessment/applicable exemption.
  • Student must self-identify to the Disability Support Services (DSS) office if accommodations are needed. DSS does NOT seek out or identify students with disabilities. Only students are permitted to request accommodations for themselves. Parents (and/or other individuals) are not permitted to request accommodations for their child.
  • Complete required DSS Student Intake e-Form under ST Disability Services (SEE IMPORTANT NOTE BELOW). All students requesting accommodations are required to complete this form electronically. Students are responsible for following up with a DSS Coordinator to ensure that our office received your e-forms. If the student fails to attach the required documentation noted on the e-form, the form will be sent back to student and will not be accepted by DSS until all required documentation has been submitted by the student. It is the responsibility of the student to contact a DSS Coordinator with any questions about the DSS e-forms. Students needing assistance with completing/submitting the DSS Student Intake e-Form may come to the DSS office for assistance with the e-form.
    • *IMPORTANT NOTE: If you are a student who is already registered with Disability Support Services (DSS) and currently or previously (within the last 3 years) received classroom accommodations through our office with your DSS Coordinator, you do not have to complete the Student Intake e-Form. Only new students who have not previously registered with DSS and have not received classroom accommodations from DSS are required to complete the DSS Student Intake e-Form. This form only needs to be completed once - this form does not need to be completed each term.
  • Provide current and appropriate medical documentation in accordance with federal postsecondary disability laws. Please see the Documentation Criteria Checklist for documentation requirements. Documentation should be attached to the Student Intake e-Form. However, if the student is unable to attach the documentation to the DSS Student Intake e-Form, the student may fax the required medical documentation to 254-526-1700 or dropped off medical documentation in person at the DSS office.  It is the responsibility of the student to follow up with DSS concerning his/her documentation. Students are responsible for obtaining and submitting all required medical documentation. High school paperwork (i.e. IEP and/or ARD) is NOT considered sufficient documentation for college level courses and will not be accepted as documentation. Some disabilities may require yearly updates on medical documentation be provided to DSS (i.e depression, bipolar disorder, anxiety, etc.). No accommodations will be provided until the student has completed the DSS Intake Process, submitted the required documentation, and requested accommodations in writing to his/her DSS Coordinator.
  • Read/adhere to all DSS policies set forth in the DSS Student Procedures Manual (this manual is located under the Student Responsibilities link).
  • Accommodations cannot be provided by the DSS Coordinator until AFTER the student has registered for classes (Accommodation Forms require the instructor's name and course number) and has formally requested accommodations from his/her DSS Coordinator (Online Accommodation Request Form - see below).
After the student has completed the required DSS Student Intake e-Form and has had his/her required medical documentation reviewed and approved by a DSS Coordinator, the student is then required to complete and submit the Online Accommodation Request Form.
  • Students who want accommodations must submit their request for accommodations EVERY SEMESTER AFTER REGISTERING FOR CLASSES by completing and submitting the Online Accommodation Request e-Form found under ST Disability Services to his/her assigned DSS Coordinator. Student is also responsible for reporting any schedule and/or instructor changes to his/her DSS Coordinator once an Accommodation Letter has been given. DSS does not send out accommodation reminders to students. If you need assistance with this form, please contact the DSS office at 254-526-1195 or contact your assigned DSS Coordinator.  
  • The student will ONLY receive reasonable classroom accommodations that he/she qualifies for based on his/her specific documented disability. Final determination of approved accommodation lies with the DSS office and its agents.
  • Students may request accommodations anytime during the term; however, DSS strongly recommends that students request their accommodations immediately upon completion of registration and providing instructors with the Accommodation Letter as soon as possible because accommodations are not retroactive. Students have the right to utilize or not utilize their approved accommodations.
  • Approved accommodations will only be provided for the registered courses that the student requests accommodations for; however DSS strongly recommends that students request accommodations for all their courses.
  • The student is responsible for communicating with instructors about the utilization of the approved accommodations for all courses.
  • Accommodations go into effect once the instructor(s) receives the Accommodation Letter(s) from the student’s assigned DSS Coordinator. It is the responsibility of the student to follow up with course instructors to ensure the instructor has received a copy of the student’s Accommodation Form from the DSS Coordinator.
*DSS Coordinators will email Approved Accommodation Forms to instructors and students within 3 to 5 business hours AFTER the student has requested his/her accommodations for the term's registered classes by completing and submitting the Online Accommodations Request Form (students must request accommodations every term with their Disability Coordinator). All Accommodation Forms will be emailed to the student's school email address (CTC's Eagle email address).
*Students located outside of the service area may contact a DSS Coordinator directly for assistance.
*It is the responsibility of the student to directly contact his/her DSS Coordinator to discuss any questions, concerns, or problems regarding his/her accommodations and/or DSS policies (problems should be reported immediately as it may be too late to address the problem if the students does not report it in a timely manner or waits until after the course has ended).
*Students are responsible for monitoring their own academic progress and are encouraged to seek free tutoring at the Academic Studio (accommodations allow students equal access and does NOT guarantee academic success or passing grades).
*If a student fails to request accommodations, DSS, the course instructor, nor CTC is obligated to provide accommodations until the student has formally requested accommodations by completing and submitting the required Online Accommodations Request Form. Please note that accommodations are NOT retroactive.
