Transcript Request

If you need a copy of your OHS transcript, contact us directly at (254) 526-1592 or at the following e-mail address, graduation.info@ctcd.edu.  Upon verification of your attendance, you will be directed to CTC eforms, where there is a CTC Online High School Transcript Request form to fill out.  This form is filled out and sent electronically.

There is no fee for an OHS transcript. Once your request is received, it will be sent to you via mail in two business days. If you have any questions in reference to this process, contact CTC Graduation Office at the e-mail provided above or call 1-800-792-3348, ext. 1592.
